Stroke risk can build silently for years. Learn how blood pressure, cholesterol, blood sugar, smoking, weight, sleep, and activity affect stroke risk—and when carotid ultrasound or other vascular testing may be appropriate.

Stroke Prevention Starts Long Before Stroke Happens

Most people think of stroke as something that happens suddenly, without warning. In reality, the vascular damage behind a stroke usually builds for years — through untreated high blood pressure, unmanaged blood sugar, elevated cholesterol, smoking, inactivity, and poor sleep. The encouraging part: a large share of first strokes are preventable when these risk factors are caught and treated early.

At Complete Neurological Care, our providers work with patients across our Boynton Beach, Boca Raton, Coral Springs, Forest Hills, Broadway, Midtown, Valley Stream, Hicksville, and Passaic offices. The patients who do best are rarely the ones who wait for a warning sign — they're the ones who start managing vascular health before anything goes wrong.

The Risk Factors That Matter Most

Modifiable stroke risk comes down to a short list, and most of it is manageable with the right care plan:

  • High blood pressure — the single biggest modifiable driver of first stroke
  • Diabetes and insulin resistance — high blood sugar damages blood vessels over time
  • High cholesterol — contributes to plaque buildup in the arteries feeding the brain
  • Smoking and inactivity — both accelerate vascular damage
  • Poor sleep and excess weight — increasingly recognized as core vascular risk factors, not just "wellness" concerns

Addressing these isn't optional extra credit — it's the foundation of modern stroke prevention.

Lifestyle Changes With Real Impact

Small, consistent changes add up:

  • Aim for at least 150 minutes of moderate exercise per week, plus strength training
  • Follow a Mediterranean-style eating pattern
  • Quit smoking — at any age, this measurably lowers risk
  • Prioritize sleep quality, not just quantity
  • Keep blood pressure, blood sugar, and cholesterol at your provider's target levels

When Vascular Imaging Is Actually Necessary

This is where a lot of confusion comes in. Routine carotid ultrasound isn't recommended for every healthy adult — for most people, the downsides of unnecessary testing outweigh the benefit. But imaging becomes important for patients who already have multiple risk factors, known cardiovascular disease, or diagnosed carotid narrowing. In those cases, our neurodiagnostic testing team may recommend periodic carotid duplex ultrasound, and in select patients, transcranial Doppler to monitor blood flow deeper in the brain's circulation.

The distinction matters: we don't screen everyone, but we closely monitor the patients who need it.

Frequently Asked Questions

Can stroke really be prevented?
In most cases, yes. The majority of first strokes are linked to modifiable risk factors like blood pressure, blood sugar, cholesterol, and smoking — all of which can be identified and managed before a stroke occurs.

Do I need a carotid ultrasound?
Not necessarily. Routine screening isn't recommended for the general population, but it may be appropriate if you have multiple vascular risk factors, known heart or blood vessel disease, or a prior finding of carotid narrowing. Your provider can help determine if this applies to you.

What's the single most important thing I can do?
Get your blood pressure checked and controlled — it's the leading modifiable risk factor for first stroke.

When should I see a neurologist about my stroke risk?
If you have high blood pressure, diabetes, high cholesterol, or a family history of stroke, an evaluation with one of our neurologists can help you build a personalized prevention plan before symptoms ever appear.
